What is a Questionnaire?

This module allows you to conduct surveys with students in any format.

10.6.1. How to Create a Questionnaire

1. Install the survey module.

2. Configure the survey settings and click "Save"

3. Click "Questions" to create a survey question.

4. Select the question type from the drop-down menu under "Add question" Click "Add selected question type" to add the question.

5. You can reorder, edit, or delete the questions you’ve added under “Manage Questions.”

10.6.2. Questionnaire Settings

<General>

  • Name: This is the title of the survey.

  • Description: This is the description for the survey.

<Availability>

  • Allow responses from: The date the survey begins.

  • Allow responses until: The date the survey ends.

<Response Options>

  • Type: Select the number of times the survey can be taken.

  • Respondent Type: Set whether respondents will answer anonymously or by name.

  • Students can view all responses: Configure whether students can view survey responses and, if so, when they can do so.

  • Send submission notification: Set whether to notify the instructor when a response is submitted.

  • Save/Resume amswers: Set whether responses can be saved before submission.

  • Allow branching question: Set whether questions should branch based on the response.

  • Automatic Numbering: Set whether to automatically assign numbers to questions.

  • Display progress bar: Displays a progress bar to indicate progress.

  • Submission Grade: Set whether to assign a grade upon submission.

Items that can be configured in "Advanced Settings"

<Advanced settings> - Subtitle: Set the subtitle that appears below the title on the first page.

  • Additional info: This is the text displayed above the title on the first page of the survey.

<Submission Options> - Confirmation URL: Set the URL to which users will be redirected after completing the survey.

  • Heading Text: Set the header for the page displayed after the survey is completed. This option is enabled if no confirmation URL has been set.

  • Body text: Set the text that appears on the page displayed after the survey is completed. If left blank, the message "Thank you for completing this survey" will be displayed. This option is enabled only if no confirmation URL has been set.

10.6.3. Question Types You Can Add to a Survey

  • Page break: Inserts a page break at the question.

  • Yes/No: A question that requires a "Yes" or "No" answer.

  • Slider: A question where you select an answer by sliding a slider.

  • Checkbox: A question that requires you to select an answer using a checkbox.

  • Text box: This is a question where you enter a short answer in the text box.

  • Dropdown box: This is a question where you select an answer from a drop-down menu.

  • Numeric: This is a question that requires a numerical answer.

  • Date: This is a question where you must enter or select a date to answer.

  • Rating (scale1–5): This is a question where you rate each item on a scale.

  • Essay box: This is a question that allows you to provide a detailed written response.
